Healthcare & Facility-Based Notarization

Professional notarization for estate planning and real estate documents conducted within healthcare and senior living environments when signers are unable to attend a law office or title company.

Services are coordinated directly with attorneys, title companies, and professional staff to ensure proper execution in structured facility settings.

I am experienced in working within structured facility environments and coordinating directly with staff to ensure efficient, compliant document execution.

Appointments are conducted with a calm, structured approach designed to support the signer’s comfort while maintaining proper execution standards.

Healthcare & Senior Living Signings

Facility Environments Served

Assisted Living & Memory Care

Notarization services for residents of assisted living communities and memory care facilities when documents must be executed outside the attorney’s office.

Skilled Nursing & Rehabilitation Facilities

Signings coordinated with facility staff when residents require bedside or in-room notarization.

Healthcare & Care Environments

Appointments may also occur in hospitals, hospice facilities, dialysis centers, or other healthcare settings when a signer cannot travel.

Facility Coordination

How I Work With Your Team

When a signing is scheduled at a healthcare facility, I coordinate with the designated facility contact to confirm access procedures, resident availability, and signing logistics. This allows the appointment to proceed smoothly while minimizing disruption to staff and residents.

Witness Requirements

Some estate planning and healthcare documents require witnesses in addition to notarization. When witnesses are required, they must be provided by the client, attorney, or facility staff.

I do not provide witnesses.

Witnesses must be impartial individuals and may not be beneficiaries of the document.

Documents Notarized at Facilities

What I Notarize

  • Powers of attorney — durable, medical, and financial

  • Healthcare directives and advance directives

  • Trust documents and trustee designations

  • Wills and testament execution for residents

  • Affidavits and sworn statements

  • Medicaid and benefit assignment documents

  • Admission and consent documents requiring notarization

  • Any document requiring a Texas notarial act

Signer Requirements (All Appointments)

The following conditions must be met at the time of the appointment in order for notarization to proceed:

• The signer is alert, willing, and able to communicate directly with the notary
• The signer has valid government-issued photo identification available
• Any required witnesses are present at the appointment
• Facility staff permit visitor access for the scheduled appointment

Texas law requires the notary to verify the identity of the signer and confirm that the signer appears willing and aware of the document being signed.

If these requirements cannot be met at the time of the appointment, the notarization cannot be completed.
